Facilities Management Information Center (FMIC)


 

The DSF Facilities Management Information Center (FMIC) provides centralized support for DSF's Bureau of Facilities Management.  Specifically, FMIC provides the following services:

  • Call Center -- Provide tenants centralized facility maintenance support for all DOA-owned facilities
  • Work Order Dispatching -- Dispatch work order requests (preventative and corrective) from tenants to facility management shops
  • CMMS -- Provide computerized maintenance management system (CMMS) software support to record all work order data, including status and cost to complete.

The information center is staffed weekdays from 7:30 am until 4:30 pm.  Emergency situations such as safety hazards, burning smells, leaks, plugged toilets, etc should be reported directly to the information center by calling (608) 266-1485.  If there is an emergency situation after hours, please report it to the Capitol Police at (608) 266-7700.  All other requests for building services should be made through the web-based form.

  • What's New - Find out what enhancements, changes, etc have been made to the web-based request or process.
    • The agency contact list has been updated as of November 7, 2012.  Please review this list to make sure we have all your contact information correct.  Please let us know if there are any errors, omissions or if there are people on the list that no longer work for your agency.
  • Instructions - When do I have to fill out a request for buildings services and how do I do it?
  • Request for Building Services - Web-based form for agency contacts to request building services for DOA owned facilities.  Requires IAM username/password.
  • Form 8181 - Request for Building Services - This is the old form that was being used before the web-based form was introduced.  We will allow tenants to access this form for internal use if you so desire.  This form can not be submitted to the call center or building manager in lieu of the web-based form.
  • Identity and Access Management (IAM) - Web page for activating/managing your Enterprise IAM account. Your password expires every 60 days.
  • Agency Contact List - Only agency contact designees can submit building services requests.  Check this list to see who your contact is.
  • Process Flowchart - Curious what happens to your request after you submit it - take a look.
  • Billable Services - A general list of services you are charged for.
  • Labor Charge-Back Rates - Labor rates charged for tenant requested maintenance and construction work.
  • Key Requests - How do I request a key be made.
  • DOA Owned Building Tenant Manual - Rules and information for State Agency Building Occupants in State-owned and managed buildings and facilities.
  • Facilities Managers  - Find out who is responsible for what DOA owned buildings.
  • CMMS Reports - (for DSF staff only) Backlog and other reports related to DOA Building Maintenance Work Orders.
