The
The information center is staffed weekdays from 7:30 am until 4:30 pm. Emergency situations such as safety hazards, burning smells, leaks, plugged toilets, etc should be reported directly to the information center by calling (608) 266-1485. If there is an emergency situation after hours, please report it to the Capitol Police at (608) 266-7700. All other requests for building services should be made through the web-based form.

Request for Building Services - Web-based form for agency contacts to request building services for DOA owned facilities. Requires IAM username/password.
Form 8181 - Request for Building Services - This is the old form that was being used before the web-based form was introduced. We will allow tenants to access this form for internal use if you so desire. This form can not be submitted to the call center or building manager in lieu of the web-based form. Identity and Access Management (IAM) - Web page for activating/managing your Enterprise IAM account. Your password expires every 60 days.
Agency Contact List - Only agency contact designees can submit building services requests. Check this list to see who your contact is.
